ASSIGNMENT OF SAVINGS ACCOUNT
For value received, I, (1), of (2), transfer and assign to (3), of (4), as assignee, savings account No. (5), in my name with (6), and all moneys due or to become due thereunder. I authorize (7) to pay over to assignee this sum out of moneys deposited in my name in the above-mentioned account.
